I, the undersigned, am an applicant for volunteer work at Church of the Lamb (the “Church”) and have been advised that as a part of the application process, the Church conducts a criminal history background check, as well as a personal reference check. I consent for the Church to use any information that is provided in this application to perform this criminal history check and to contact my personal references to obtain information that the Church might consider relevant to my application. The Church has informed me that I have the right to review and challenge any negative information from the criminal history check that would adversely impact a decision to offer volunteer work. In addition, I have been informed that I will have a reasonable opportunity to clear up any mistaken information reported within ninety (90) days of this application. Information received from personal references will remain confidential. I also understand that the Church may discontinue my volunteer work at any time, and for any reason. When volunteering, I will not be an employee of the Church. By signing below, I affirm that all the information I have provided in my application is true, correct, and complete, and does not omit any information necessary to prevent the statements in my application from being misleading.
